HIPAA governance evidence

VeriProof can record AI activity, reviews, and integrity evidence. Your HIPAA risk analysis determines which records and safeguards are required.

Do not send PHI until the required agreement and deployment controls are approved.

Define the evidence scope

Document the systems, applications, users, data, and review period in scope. Identify which VeriProof records contain electronic PHI.

The HIPAA Security Rule requires administrative, physical, and technical safeguards for electronic PHI. A VeriProof export supports selected audit and integrity controls; it does not cover the complete environment.

Record AI activity

Use the SDK to capture the events needed for the approved governance purpose. Minimize PHI before capture and test sensitive-field controls with non-production records.

Check that the records identify the action, time, application, and outcome needed for an investigation. Include the authorized reviewer’s decision where human review applies.

Review access and integrity

Confirm that role permissions limit access to the intended users. Use the configured proof provider to check exported evidence integrity.

Integrity verification detects whether exported content matches its recorded commitment. It does not determine whether a clinical decision was correct.

Prepare audit evidence

For an approved audit period:

  1. Define the application, date range, and control objective.
  2. Export only the records the reviewer is authorized to receive.
  3. Verify the export manifest and provider proofs.
  4. Record any exclusions, gaps, and follow-up actions.
  5. Store the package under your approved retention procedure.

Combine VeriProof evidence with policies, risk analyses, training records, incident records, and other evidence owned by your organization.
